To access our support services, you need to complete one of our online referral forms below.

If you are an individual wanting support with us, you need to complete our ‘Self Referral Form’. If you are a professional/organisation wanting to refer an individual into our services (with their knowledge and consent), you need to complete our ‘Professional Referral Form’.

Once we have received your referral form, a member of the team will be in touch with you within 3 working days.

Please note: both referral forms are facilitated by our Synergy East Partnership. Therefore, the logo and name you will see on the forms are Synergy East, rather than CRCC.
